If you received a text message from the Oklahoma Department of Human Services (OKDHS) about signing up for emergency food boxes, please complete the short form below.

This form is required by OKDHS to verify your information. A team member from the Food Bank of Eastern Oklahoma will contact you to arrange pickup or delivery.
