Reports are due by the 10th of each month, reporting numbers for the previous month. The information contained in the report makes it possible for us to identify the needs in our service area and help you meet those needs. This information also enables us to educate legislators and the general public about the issues of hunger and helps us raise the funds we need to operate.

Monthly reports must be completed and returned even if nothing was ordered from the Food Bank that month. Programs closed and/or not serving clients during the month must still submit a report for that month.

Programs are placed on product hold if the report is late, and in addition are assessed a late fee of $25 per month for delinquent reports, starting with the second incident. . Programs on product hold are not allowed to place or pick up orders.

Instructions for Completing Monthly Reports

The monthly report is now accessible on the Agency Express website, the same online system where your program orders food.

1)     You must be an authorized online shopper to access Agency Express, where the monthly reports are located. If not authorized, please contact Phyllis Holmes at or 918-936-4546.

2)     Next, log on to Agency Express using the username, password and program code assigned to you when you became an authorized shopper. Once you log on to the site, you will go straight to the "Food Bank Links" tab.

3)      After clicking on this tab, you will see options for reports based on the corresponding month and program type. Open the report you need to complete, fill it out and then hit submit. A few reminders, make sure to open the correct report and if you have a pantry and onsite, you must complete two reports each month.

Important! When you fill out the report, your Program ID is the 5 digit Agency Number, a dash, and then your 2 digit Program Code. Example: 12345-01.
